    If you are a Support Coordinator, Case Manager, Allied Health Professional, Parent/Caregiver, partner or other important person in the participants care you can also use this form on behalf of the person you are trying to get treatment for.

  • How To Make a Referral to WHAM

    Step 1. Ensure you have all your details handy including any doctors referrals, NDIS plan details or any other relevant documents for us to understand your needs.

    Step 2. Click the "Intake Form" link below.

    Step 3. Take your time to fill this out and use the "save" function if you are missing information so you can come back later to finish it, before you hit submit.

    Step 4. Once you have hit submit, we will receive your details and be in contact with you about when we can book in your first appointment.

NDIS

  • How WHAM Can Help NDIS Participants

    Exercise Physiology and Dietetics are available to NDIS participants who have funding allocated for these services. All fees are charged in-line with the NDIS pricing guidelines. 

    At present, WHAM can only assist you if you are self-managed or plan-managed.

    *Requires approval from NDIA under the 7 recognised reasons for accessing Disability Health Related Supports

Medicare

How Can WHAM Help You Through Medicare?

Exercise Physiology and Dietetics both can be claimed through Medicare, provided you have a referral from your GP. 

Talk to your GP today if you have a chronic disease or condition that can be managed through Exercise Physiology or Dietetic Services.

Medicare rebates are available.

  • We have 2 methods for referral:

    If your patient is not tech savvy please send through the referral form and a health summary to admin@whamcare.com.au and we will take it from there!

    If your patient is able to use technology, you can tell them to head to our intake form via the "Make a Referral" links on our site and they can upload their referral themselves and complete our intake form so we can see them ASAP!

  • We do not bulk bill so there will be a cost to your patient.

    Current prices:

    Initial consultation = $155 

    Subsequent 1 hour appointment= $120

    Subsequent 45 minute appointment = $105

    Subsequent 30minute appointment= $90

    Your patient will need to pay the full cost after the appointment and they will be rebated directly to their nominated bank account connected to their myGov account via Medicare.

  • Our Dietitian is unable to attend face to face appointments in the Shoalhaven area unless your patient is willing to attend our clinic space on a Thursday or accept a Telehealth appointment.

Department of Veterans Affairs

How Can WHAM Assist DVA Clients?

DVA will access Exercise Physiologists and Dietitians to support veterans if the therapy we provide will directly assist with your listed injury/diagnosis.

You will require a referral from your Doctor to access therapies regardless of your card colour, so speak with your GP today about whether your injury/diagnosis qualifies for additional therapeutic support.

Workers Compensation

How Can WHAM Assist You With Your Return to Work?

If you have been injured at work and require Exercise Physiology services, we can assist you. 

You will need a referral from your doctor or insurance case manager in order to begin treatment. 

Dietitians also have a role within the Workers compensation scheme, though also will require a referral to commence treatment. 

Private Payers

How Can WHAM Help You?

Exercise Physiology and Dietetics are available for people who would simply like to lead a healthier life and want support to do this. 

Doctor referrals are not required to access us, however, this does mean the full cost of the appointment is payable by you. 

We do also hope for you to know that if you have any medical conditions that place you as high risk, a medical clearance may be requested by us from your doctor before commencing with therapies.
